Customer sent pics as per below. He says that this engine is professionally made (does not look like a homebuilt) and also claims he saw another one in an OEM Cox Box (?). Ronald Valentine says that it is not one of his engines. It is .051 size with a reedie crankcase and TD intake setup (sorry, not the greatest pictures).

Professionally made...at home. Over the years there has been many hobbyist making rear intake reedies. The only one Cox ever made close to this was the Space Hopper.

That's a Tarno AR carburetor mount with the Tarno reed intake installed. It has the TD NVA and venturi installed. I built one like that for a tach race a while back.
