It has the same principle as axial piston hydraulic motors and pumps. Hydraulic pumps and motors have an odd number of pistons though... 5, 7 or 9 usually, to make sure it will start every time.

It has been done before, engines using the wobble plate principle as in hydraulic motors or pumps were around even bofore the war. I had a look in my ancient vintage car magazines: On the 1948 Paris motor show, people could discover the REINE 1950, a revolutionary car made by Mr Brandt with a little cumbersome wobble plate engine.

Wow, looks like a two stroke with counter-pistons
All kinds of strange things have been conceived in the past. I do remember reading about a Profi type engine, I think it was a single cylinder engine where the piston went back and forth inside a rotating cylinder
