Actually no but I didn't torque the wrench over that hard. It took me by surprise, and the cylinder broke loose when the wrench distorted.

It was an old TeeDee and the insides were completely brown with goo. I suspect that it was run very hot and hard during it's life.

Probably a stress crack at the dimple.
One in a million I'd guess.

You should always get into the habit of heating engine before you put a wrench to it, believe me you'll save a lot of parts..and wrenches.
